 Schedule for the 2009 Stanly County 4-H Summer Blast Program

To register your child to particapate in the 2009 Stanly County 4-H Summer Blast Program review the schedule listed in the Program link above and complete the requested information for each child on the registration form and bring with you to the Extension office in the Stanly County Agri-Civic Center. Registration must be completed in person. You can call the office (704.983.3987) to check to see if there are vacancies in the session in which you are interested.

Youth participating in the 4-H Summer Blast program must have a 4-H Enrollment Form, Photographic, Video, and Audio Optional Publicity Release, and a 4-H Medical Information and Informed Consent for treatment for 4-H sponsored events. The Informed Consent file must be notarized - there is a Notary Public in the Extension office.
